1.     I have read and understand the MILE-HI R/C BY-LAWS and SAFETY RULES, and agree to abide by them and by the AMA SAFETY CODE.

2.     I agree that I am financially responsible for damage caused by me or by my models while at the club flying field.

3.     I understand that violation of these rules and safety codes may be grounds for termination of club membership.

4.     I understand and agree that my AMA must be paid, current and in effect prior to flying at the club field.

5.     I understand upon leaving Mile-Hi R./C Club my Field Improvement Fee will be forfeited. Field Improvement Fee will also be forfeited upon my resignation or failure to renew dues.

6.     I understand that club dues are paid in advance prior to the start of the club's fiscal year on October 1st of each year.
